We have a great new system to keep all of the information we have about our members and the services / activities they want to access. This is much more secure and will allow us to provide much better support to people with a learning disability and their families.
Please help us to support you better, by updating your details and telling us what you would like us to do. We will be able to send you better information about the things you’ve told us you want help with.

- If you are a family carer please fill out this form focusing on the member of your family with a learning disability
- If there is more than one member with learning disabilities, please fill out an additional form
- Please provide all relevant family contacts
- Please feel free to attach any other information you would like us to know on a separate sheet
